Job summary

Hackensack Meridian Health in New Jersey is seeking a Care Coordinator, Care Management to coordinate, communicate, and facilitate the clinical progression of a patient’s treatment and discharge plan within the multidisciplinary care team. You will manage a designated caseload, collaborate with physicians and the care team, and arrange the appropriate next level of care.

The role requires NJ RN or Social Work licensure (or masters in social work), strong communication and clinical judgment, and

Qualifications

• BSN or BSN in progress and/or willing to acquire within 3 years of hire or transfer; or Masters Degree in Social Work.
• Strong decision-making, problem-solving, and leadership skills.
• Excellent verbal, written and presentation skills.
• Proficient with Microsoft Office and Google Suite.
• Familiar with hospital resources and community resources.

Responsibilities

• Assesses patients and develops individualized care plans with the care team.
• Facilitates communication and coordination among health care team and family.
• Maintains knowledge of community resources and refers patients as needed.
• Collaborates with post-acute and multidisciplinary teams for timely transitions.
• Documents care plans, referrals, and authorizations in medical records.
