Concierge Home Care in Clearwater is seeking a Clinical Team Support (Scheduling Coordinator) to keep our clinicians on track and patients well cared for. The role coordinates visits, aligns with physician orders, and ensures timely communication between patients and staff.

This in-office position requires organization, strong communication, and the ability to adapt to a fast-paced team. Prior home health scheduling experience is preferred, with opportunities for growth and ongoing training.

Qualifications

• High school diploma is required.
• Prior home health scheduling experience is required.
• Experience in home health administration is preferred.
• Strong organizational and multitasking skills.

Responsibilities

• Scheduling patient visits in accordance with physician orders and care plans.
• Coordinating clinician schedules to optimize productivity and coverage.
• Managing daily scheduling changes, call-outs, and urgent visit needs.
• Answering and triaging incoming calls to support clinical operations.
• Tracking medical supplies and supporting RAP/EOE processes.
• Maintaining accurate scheduling and documentation within the EMR.
• Supporting the clinical team with administrative tasks as needed.

Your Role as Clinical Team Support

In this role, you will support daily clinical operations by coordinating schedules and ensuring seamless communication between patients and clinicians.

Responsibilities include:

• Scheduling patient visits in accordance with physician orders and care plans
• Coordinating clinician schedules to optimize productivity and coverage
• Managing daily scheduling changes, call-outs, and urgent visit needs

Schedule: Monday–Friday, 8:30 AM – 5:00 PM (in-office role)

Compensation: negotiable, based on experience
