Job summary

Elara Caring is seeking a Clinical Team Manager in Akron, Ohio. This role combines hands-on clinical knowledge, team coaching, and care coordination, partnering with branch leadership to support nurses and clinicians who rely on your guidance.

You will guide clinicians, solve care challenges, and help patients receive quality care at home, bringing together nursing leadership and operations to ensure excellent patient outcomes.

Qualifications

• Associate degree in nursing or a related field.
• RN license must be current and unrestricted in Ohio.
• At least 2 years of home care RN experience in home health or hospice.
• 1 year of supervisory experience preferred.
• Proficiency in Homecare Homebase and Workday; working knowledge of Microsoft Office.

Clinical Team Manager, RN, Home Health Akron, Ohio | Elara Caring You know home health. Now help lead the team delivering it. Your next step in nursing could be guiding clinicians, solving care challenges, and helping patients receive quality care at home. Elara Caring is seeking a Clinical Team Manager in Akron, Ohio. This role brings together hands-on clinical knowledge, team coaching, and care coordination. You'll partner with branch leadership and support the nurses and clinicians who count on your guidance.

What You’ll Need

• Associate degree in a nursing-related field.
• At least 2 years of home care experience as an RN in home health or hospice.
• Current, unrestricted RN license valid in Ohio.
• 1 year of supervisory experience preferred.
• Ability to quickly become proficient in Homecare Homebase and Workday, with working knowledge of Microsoft Office.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• A valid driver's license and acceptable driving record, OR reliable transportation to travel between patient homes and assigned work locations.
