Job summary

Elara Caring is seeking a Clinical Team Manager in Texas to lead patient care services, coordinate Home Health operations, and ensure regulatory compliance. You will oversee staff, plans of care, and provider relations while driving quality and efficiency.

You will guide a team of clinicians, perform OASIS reviews, and collaborate with the Branch Director to monitor key performance metrics across the clinical operation.

Qualifications

• Associate degree in a nursing-related field.
• 2 years home care RN experience in Home Health or Hospice.
• 1 year supervisory experience preferred.
• Current, unrestricted RN license for the state of work.
• Ability to quickly become proficient in Homecare Homebase, Workday, and have knowledge in Microsoft Office.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.
• Must possess a valid driver's license or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Provide guidance and coaching to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor KPIs and support clinical operations.

What is Required?

• Associate degree in a Nursing related field required
• 2 years home care experience as a registered nurse in a Home Health or Hospice environment
• 1 year of supervisory experience preferred
• Current, unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
