Job summary

Eventus WholeHealth is seeking a Clinical Support Specialist to work with the care team, coordinate workflows, and support clinical services in skilled nursing and assisted living facilities. You will aid with charting, schedules, telehealth, and patient communication in a fast-paced healthcare environment.

Ideal candidates have clinical care experience, strong organizational and communication skills, and a valid driver's license for travel to facilities.

Qualifications

• High school diploma or equivalent.
• Certified/Registered Medical Assistant, CNA, or PCT highly preferred.
• Valid driver’s license and reliable transportation.
• Strong customer service skills and professional communication.
• Ability to navigate multiple priorities in a dynamic clinical environment.

Responsibilities

• Prepare for daily clinical rounds with complete patient information and schedules.
• Assist clinicians with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain patient schedules, including admissions, discharges, and changes.
• Act as liaison between clinicians, facility staff, patients, and families to support communication.
• Assist with triage needs and clinician-directed documentation.
• Organize and manage clinical documentation including labs and records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and route patient consents and documentation.
• Participate in facility meetings and relay updates to the care team.
• Support discharge planning and census accuracy.
• Maintain professionalism within partner facilities.
