Job summary

Eventus WholeHealth is seeking a Clinician Support Specialist to coordinate workflows and support clinical services in skilled nursing and assisted living facilities. You will work with Advanced Practice Clinicians and facility staff to enhance communication among patients, providers, and care teams.

The role requires strong organization, attention to detail, and the ability to perform in a fast-paced healthcare environment with regular travel to assigned facilities.

Qualifications

• High school diploma or equivalent.
• Certified/Registered Medical Assistant, CNA, or PCT preferred.
• Minimum one year of patient care experience preferred; medical scribing experience preferred.

Responsibilities

• Prepare for daily clinical rounds with patient information, documentation, and schedules.
• Assist clinicians with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain accurate patient schedules including admissions, discharges, and changes.
• Serve as liaison between clinicians, facility staff, patients and families to support communication and quality care.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation, including lab results, diagnostics, and patient records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and route documentation appropriately.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate census and records.
• Maintain professionalism and positively represent the organization in partner facilities.
