Park Place Health and Wellness Center in St. John, IN is seeking a Clinical Support Licensed Practical Nurse Manager (LPN) for full-time long-term care operations.

You will oversee LTC clinical operations, assist with memory care and ensure compliance with regulations, audits, and infection control programs. The role requires Indiana LPN licensure, prior management or scheduling experience in skilled nursing, and the ability to lead daily rounds and participate in committees.

Qualifications

• Must be a licensed practical nurse (LPN) in Indiana.
• Prior management/supervisor or scheduling experience in skilled nursing/long-term care is a plus.
• Must pass physical, drug screen, lift test, background check, and orientation.

Responsibilities

• Assist LTC Clinical Operations in Assisted Living, Memory Care and Long-Term Care.
• Ensure daily operations meet federal, state and local standards and regulations.
• Assist QA&A Committee in action plans to correct deficiencies.
• Complete audits and Infection Control program in the building.
• Schedule daily rounds to observe clients and nursing staff and ensure quality care.
• Provide reports/recommendations to the Director of Nursing as required.

Summary of essential job functions:

• Assist LTC Clinical Operations in Assisted Living, Memory Care and Long-Term Care.


• Ensure the daily operations are in accordance with current federal, state and local standards, guidelines and regulations.


• Assist the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.


• Complete necessary audits and Infection Control program in the building.


• Schedule daily rounds to observe clients and nursing staff, ensuring that quality care is provided to clients.


• Make written and oral reports/recommendations to the DON as necessary/required, concerning nursing operations.


• Participate in PLS surveys made by authorized government agencies.


• Attend various committees of facility (i.e., Infectious Control, Policy Advisory, Pharmaceutical, Budget, Quality Assessment and Assurance, etc.) as required, and as appointed by the administrator.


• Maintains master forms on computer for: Floor schedule, calendar attendance, trade slips, call-in slips, leave early slips, payroll deduction sheet, new schedule sheet, etc.


• Assist with on-boarding and training of new hires.


• Take and make phone calls after hours.


• Work the floor when necessary; and every other weekend on call. Some holidays on call.


• Reports to Director of Nursing.

Requirements

• Must be a Licensed Practical Nurse (LPN) in the State of Indiana.


• Prior experience as a manager/supervisor or scheduling with skilled nursing/long-term care a plus.


• Pass a physical, drug screen, 50-pound lift test, fingerprint for criminal background and complete the orientation program.
