Job summary

Park Place Health and Wellness Center in St. John, IN is seeking an LTC Clinical Support Nurse Manager (RN) for a full-time role.

You will oversee daily clinical operations across assisted living, memory care, and long-term care, ensuring compliance with federal/state guidelines and coordinating with the DON. The position requires Indiana RN licensure and prior management or scheduling experience in skilled nursing/long-term care.

Qualifications

• Must be a Registered Nurse (RN) in Indiana.
• Prior management, supervisory, or scheduling experience in skilled nursing/long-term care is a plus.
• Must pass physical, drug screen, lifting test, background check, and orientation.

Responsibilities

• Assist LTC Clinical Operations in Assisted Living, Memory Care and Long-Term Care.
• Ensure daily operations comply with current federal, state and local standards, guidelines and regulations.
• Assist the QA&A Committee in developing and implementing corrective action plans for deficiencies.
• Complete audits and Infection Control program in the building.
• Schedule daily rounds to observe clients and nursing staff, ensuring quality care.
• Provide written and oral reports/recommendations to the DON as required.
• Participate in surveys conducted by government agencies.
• Attend facility committees as required and appointed by the administrator.
• Maintain master forms on computer for scheduling, attendance, payroll deduction etc.
• Assist with onboarding and training of new hires.
• Take and make after-hours calls; work on the floor when needed; every other weekend on call; some holidays on call.
