Job summary

Parker Health and Rehabilitation Center is seeking Licensed Practical Nurses to provide daily care and health assessments for residents in our 80-bed skilled nursing home. You will document health status, assist with care plans, and supervise nursing staff to ensure high-quality care.

We offer a $5,000 sign-on bonus for PT and a $10,000 sign-on bonus for FT, competitive wages, and benefits including medical, vision and dental, tuition reimbursement, and a daily pay option.

Qualifications

• Licensed Practical Nurse with an active Indiana LPN license.
• Understanding of safe, effective nursing practices in Indiana.
• Basic computer skills including Excel and Word.

Responsibilities

• Provide daily care, health needs, safety and comfort for residents.
• Assess and document residents health; contribute to care plans.
• Supervise and assign care tasks; support clinical staff.
• Observe for medication responses; record vitals and I&O.
• Assist with staff orientation and ongoing training.

Licensed Practical Nurse (LPN) Primary Responsibilities

The LPN is responsible for providing for the daily care, health needs, safety and comfort of the residents. Including providing professional assessments and documentation of the residents health and assisting with the management of the assigned nursing units staff.

Licensed Practical Nurse (LPN) Essential Functions

• LPN’s will supervise, perform and provide written/verbal care assignments, routine skills and procedures to ensure the quality of care to residents
• Participate in the development and implementation of resident care plans, maintaining up to date nursing care plans, documenting progress and making recommendations for revisions to care plans according to facility policy and procedure
• LPN’s will make daily and periodic visits to observe for symptoms, reactions to medication and/or treatment
• Obtaining and documenting vital signs, intake and output, weights, diet supplements and replacements, cultures, etc.
• Assisting with the orientation and on-going training of staff to assure required skill levels are maintained for all staff
• Reporting and documenting resident and/or visitor incidents, accidents, complaints or allegations according to the facility’s policy and procedure using the designated occurrence reporting system

Licensed Practical Nurse (LPN) Qualifications

• A graduate of an accredited school of nursing and possess a valid LPN License in good standing according to State and Federal requirements
• Have a thorough understanding of the principles of safe effective nursing practices within the State of Indiana
• Have basic computer skills along with knowledge of such programs as Excel, Word, etc.
• Personal integrity and the ability to deal tactfully with residents, family members, visitor, government agencies and personnel
