Job summary

CareStar, Inc. in Ohio is seeking a Clinical Supervisor for the SRS program. This role offers full-time employment with remote work and field visits, focusing on coordinating care for individuals across the state.

The ideal candidate will be a licensed RN or licensed social worker with extensive experience in home- and community-based services, plus supervisory experience and CCM certification or plan to obtain it within a year. Strong assessment, planning and coordination skills are essential.

Qualifications

• Must be licensed RN, LSW/LISW with at least five (5) paid years in home-and community based services and a minimum of one (1) year of supervisory experience.
• CCM certification required or obtained within one year of being in the position.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
