Job summary

CareStar, Inc. is seeking a Clinical Supervisor for the Ohio Home Care Program. The role combines remote work with required field visits to Sandusky County and surrounding areas.

You will oversee care coordination, assess client needs, and develop personalized care plans to connect individuals with essential services. Qualified candidates are licensed RNs, LSWs or LISWs with 5+ years of clinical home- and community-based experience and CCM certification within one year.

Qualifications

• Must be licensed RN, LSW or LISW with 5+ paid years in home- and community-based services.
• CCM certification required or obtained within 1 year of start date.
• Experience completing assessments, service plan development and monitoring.

Minimum Qualifications

• Must be either licensed RNs, licensed social workers or licensed independent social workers (LSW, LISW) with at least five (5) paid years of clinical experience in home-and community based services and a minimum of one (1) year of supervisory experience.
• Certified Case Manager (CCM) required or obtained within one year of being in the position.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
• Experience providing care management/coordination preferred.
