CareStar, Inc. seeks a Clinical Supervisor for the OhioHome Care Program to lead clinical operations in Allen County and surrounding areas. This role involves supervising staff, guiding assessments, and coordinating care plans with community resources.

You will work remotely with field visits, ensuring adherence to policies, maintaining patient confidentiality, and delivering high-quality home-based care coordination across Ohio.

Qualificações

• Licensed RN or social work license with 5+ years in home and community based services.
• Minimum 1 year of supervisory experience.
• CCM certification required or obtained within 1 year of start.
• Experience in completing assessments and service plans.
• Strong organizational and communication skills.

Key Responsibilities

• Maintains confidentiality as related to patient information.
• Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

Minimum Qualifications

• Must be either licensed RNs, licensed social workers or licensed independent social workers (LSW, LISW) with at least five (5) paid years of clinical experience in home-and community based services and a minimum of one (1) year of supervisory experience.
• Certified Case Manager (CCM) required or obtained within one year of being in the position.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
• Experience providing care management/coordination preferred.
