Job summary

CareStar, Inc. is seeking a Clinical Supervisor to guide the OhioHome Care Program across Guernsey County and surrounding areas. You will oversee assessments, care planning, and coordination of services while ensuring quality, compliance, and compassionate care.

You will work remotely with field visits, supervising skilled clinicians and coordinating with community resources to improve patient outcomes and independence. A valid license and CCM readiness are required.

Qualifications

• Must be licensed RN or licensed social worker with at least five paid years of clinical home-based services and a minimum of one year supervision.
• CCM certification required or obtained within one year of hire.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
• Experience providing care management/coordination preferred.

Responsibilities

• Assess clients' needs and develop personalized care plans.
• Direct and supervise field staff to ensure quality home care services.
• Coordinate services with hospitals, clinics, and community resources.
• Monitor client progress and update care plans accordingly.
• Ensure compliance with Medicare, Medicaid and waiver programs.
• Maintain confidentiality and adhere to CareStar policies.

Minimum Qualifications

• Must be either licensed RNs, licensed social workers or licensed independent social workers (LSW, LISW) with at least five (5) paid years of clinical experience in home-and community based services and a minimum of one (1) year of supervisory experience.
• Certified Case Manager (CCM) required or obtained within one year of being in the position.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
• Experience providing care management/coordination preferred.
