Be Healthy at Home is seeking a Clinical Supervisor RN to oversee nursing staff and ensure high-quality home health service delivery. You will train and mentor staff, manage active care plans, and collaborate with the Director of Nursing on discharge planning.

Ideal candidates have Texas RN licensure, CPR certification, and 2+ years in home health or case management, with a strong focus on patient satisfaction and complaint resolution.

Qualifications

• Case management: 1 year (Required).
• Home health: 2 years (Required).
• CPR Certification is required.

Responsibilities

• Oversee patient care and ensure high-quality service delivery within the home health setting.
• Educate and mentor patient care teams; facilitate discharge planning with quality outcomes.
• Participate in QAPI process and track infections and incidents for the caseload.
• Respond promptly to patient/employee complaints and concerns.
