This role requires supervising a home health team, managing patient care, and ensuring compliance with health standards. Candidates must have at least 2 years of experience in a licensed home health agency and a valid RN license. Qualifications

• 2 years of experience within a licensed and certified (Medicare) home health agency.
• Competent in Federal (Medicare) and Local standards.
• Basic level computer skills including Outlook, Word, and Excel.
• Valid RN license within practice State.

Responsibilities

• Supervise and ensure competencies of assigned home health care team.
• Provide ongoing case management of all patients' care plans.
• Provide clinical oversight for home health patients.
• Review and approve final Plan of Care within the EMR system.
• Coordinate care and clinical needs with assigned home health care team.

Skills

Experience in home health agency

Basic computer skills (Outlook, Word, Excel)

Valid RN license
