Job summary

PruittHealth Pharmacy Services Valdosta is seeking a Pharmacist to provide pharmaceutical services in post-acute and long-term care settings, ensuring accurate processing and high-quality patient care.

Qualifications

• Graduate of ACPE accredited School of Pharmacy.
• Current licensure in the state of practice.
• Long-term care pharmacy experience preferred.
• Experience in patient care, medication management, and performance improvement.

Responsibilities

• Review prescriptions to assure accuracy and determine ingredients.
• Evaluate orders for drug appropriateness, dosing, interactions, and stability; suggest changes.
• Collaborate with healthcare team to provide clinical and technical support.
• Document all communications with physicians, nurses and team members.
• Ensure accurate intake and efficient fulfillment of prescription orders.
• Maintain required documentation per federal/state regulations and policy.
• Provide and document interim Medication Regimen Reviews for new/admit patients.
• Implement and document approved therapeutic substitution orders.
• Provide clinical assessment before contacting healthcare center.
• Demonstrate leadership and accountability for pharmacy production duties.

Education
