Job summary

Qualifications

• Graduate of ACPE accredited School of Pharmacy.
• Licensed pharmacist in state of practice.
• Long-term care pharmacy experience preferred.
• Experience in patient care and medication management.
• Ability to clearly communicate medication information to professionals and the public.

Responsibilities

• Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients.
• Evaluate prescriber medication orders for appropriateness, interactions, route, and stability; make therapy recommendations.
• Collaborate with the healthcare team to provide clinical and technical support.
• Document all communication with physicians, nurses, and other team members.
• Ensure accurate intake and efficient fulfillment of prescription orders.
• Maintain required documentation per federal/state regulations and policy.
• Provide and document interim Medication Regimen Reviews for new/admit patients.
• Implement and document approved therapeutic substitution orders.
• Conduct clinical assessments prior to contacting healthcare centers.
• Demonstrate leadership and accountability for pharmacy production duties.
• Document and investigate errors and variances.
