Job summary

PruittHealth in Valdosta, Georgia is seeking a pharmacist to provide pharmaceutical services in post-acute, long-term care, or retail settings, ensuring high quality patient care and service. The role oversees prescription processing and daily activities of the closed-door long-term care pharmacy.

Candidates should be ACPE-accredited graduates with active Georgia licensure; long-term care experience preferred.

Qualifications

• Graduate of ACPE accredited School of Pharmacy.
• Current licensure in the state of practice.
• Long-term care pharmacy experience preferred.

Responsibilities

• Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed.
• Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability; propose therapy changes as needed.
• Collaborate with the healthcare team to provide clinical and technical support as needed.
• Document all communication with physicians, nurses, and other health-care team members.
• Take responsibility for accurate intake and efficient fulfillment of prescription orders.
• Obtain and maintain required documentation per federal and state regulations and Pharmacy Services policy.
• Provide and document interim Medication Regimen Reviews for new admitted patients.
