Job summary

Qualifications

• Graduate of ACPE accredited School of Pharmacy.
• Current licensure in the state of practice.
• Long-term care pharmacy experience preferred.
• Experience in patient care, medication management and drug therapy monitoring.
• Clear communication of medication information to professionals and the public.

Responsibilities

• Review prescriptions to assure accuracy and determine needed ingredients.
• Evaluate medication orders for appropriateness, interactions, dosage, and administration.
• Collaborate with healthcare team to provide clinical and technical support.
• Document all communication with physicians, nurses and others.
• Ensure accurate intake and efficient fulfillment of prescription orders.
• Obtain and maintain required documentation per regulations and policy.
• Provide and document interim Medication Regimen Reviews for new admits.
• Implement and document approved therapeutic substitution orders.
• Provide clinical assessment before contacting the healthcare center.
• Demonstrate leadership and accountability for pharmacy production duties.
• Document and investigate errors and variances.
