Job summary

Dartmouth Health is seeking a per diem Pharmacist to support our hospital team in Vermont. You will dispense medications, monitor therapies, and collaborate under protocol to optimize patient outcomes.

Responsibilities include gathering medication histories, performing comprehensive reviews, identifying adverse events, and educating patients and staff. A VT license and PharmD or Doctor of Pharmacy are required; hospital experience preferred.

Qualifications

• Bachelor's degree in Pharmacy or Doctor of Pharmacy required.
• Valid Vermont State Pharmacist License required.
• Prior hospital pharmacy experience preferred.
• Strong written and verbal communication skills essential.

Responsibilities

• Dispense medications and monitor drug therapies.
• Complete comprehensive medication reviews and reconcile medications as applicable.
• Identify and record adverse drug reactions and adherence barriers.
• Consult and educate patients and staff about drug therapies.
• Lead or supervise technicians to ensure accurate and efficient pharmacy operations.
