Job summary

PACS is seeking a Speech-Language Pathologist to evaluate and treat patients, communicate with families and physicians, and maintain comprehensive documentation. The role involves supervising CFYs, staff training, and active participation in care conferences, with strict adherence to state practice acts and payer requirements.

The position emphasizes teamwork, documentation accuracy, and discharge planning, supporting post-acute healthcare across facilities.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology (SLP).
• Licensed as an SLP by the state where services are provided or meet state alternative requirements.
• Proficient computer skills for data entry in Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours of referral or 1 business day.
• Develop and obtain approval for treatment plans.
• Treat patients per physician plan and document progress.
• Supervise CFY/CFY‑RFP SLPs and assist staff with training.
• Communicate patient progress with supervisor and health team.
• Participate in care conferences and rehabilitation meetings.
• Record evaluations and treatment notes per state and payer requirements.
• Assist with discharge planning and caregiver training.
