Job summary

Medical University of South Carolina is seeking a Social Worker (MSW) I to support discharge planning and transition of care. Under supervision, you will assess patients and caregivers, coordinate with the health care team, and identify post-discharge risks to prevent negative outcomes.

You'll work 40 hours weekly in a hospital setting, with a $2,500 sign-on bonus. Master’s in Social Work and active MSW licensure in SC are required; Case Management certification is preferred.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• Current and active MSW licensure issued by the SC Board of Social Work Examiners is required at time of hire.
• Certification in Case Management is preferred.

Responsibilities

• Coordinate the discharge process to ensure a successful transition with the health care team.
• Complete patient and caregiver assessments during hospitalization and follow-up as needed.
• Serve as liaison between medical/nursing staff, patients, and outside agencies; interpret social problems related to medical conditions.
