Job summary

Ochsner LSU Health System in Shreveport, LA seeks a Medical Social Worker to conduct bio-psychosocial assessments, coordinate discharge plans, and connect patients and families with community resources. Qualifications

• MSW from CSWE-accredited program.
• CSW or RSW certification required or verifiable.
• LMSW licensure preferred or must schedule LMSW exam within 6 months of employment.
• Ability to perform data entry and retrieve information using computer systems.
• Strong verbal and written communication to interface with patients, families, and team members.

Responsibilities

• Conducts patient assessment interviews, provides referrals and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records and documents and communicates critical information to the medical team.
• Maintains professional knowledge and required certifications through continuing education.
• Utilizes management resources.
• Participates in on-call rotation.
• Adapts communication and service explanations to diverse patient populations.
