Job summary

Candidates should have an MSW from CSWE-accredited program, CSW/RSW certification, and LMSW as preferred.

Qualifications

• MSW from CSWE-accredited program
• CSW/RSW certification required
• LMSW preferred or must schedule LMSW exam within 6 months of employment
• Ability to document and manage patient records accurately

Responsibilities

• Conduct patient assessment interviews, provide referrals and coordinate discharge plans.
• Identify and provide helpful resources to patients and families.
• Maintain patient records and communicate critical information to the medical team.
• Maintain professional knowledge and required certifications through continuing education.
• Utilize management resources.
• Participate in on-call rotation.
• Adapt communication and service delivery to patient populations with privacy and cultural sensitivity.
