You will collaborate with a multidisciplinary team to address emotional, social, cultural, and spiritual needs and coordinate community resources.

Required: MSW from CSWE; LMSW preferred or must schedule licensure within 6 months. CSW or RSW certification required with licensure exam within 6 months.

Qualificações

• Master's degree in Social Work (MSW) required.
• LMSW licensure preferred or must be scheduled within 6 months of employment.
• CSW or RSW certification required with licensure exam within 6 months.

Responsabilidades

• Conducts patient assessment interviews, provides referrals and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records and communicates critical information to the medical team.
• Maintains professional and technical knowledge through continuing education.
• Participates in on-call rotation.
• Adapts communication to patient population with respect for privacy and cultural needs.
