Job summary

Qualifications

Responsibilities

• Conducts bio-psychosocial assessments and evaluates patient needs.
• Coordinates discharge plans and referrals to community resources.
• Maintains patient records and communicates with the multidisciplinary team.
• Keeps up professional knowledge via continuing education and certifications.
• Utilizes management resources and participates in on-call rotation.
• Adapts interactions to patient population while respecting privacy and consent.
• Performs other related duties as required.
