Qualifications

• Master’s degree in Social Work (MSW) from a CSWE-accredited university.
• Certified Social Worker (CSW) or Registered Social Worker (RSW) per LABSWE; LMSW preferred with exam plan.
• Must be proficient with Windows-based applications and data entry.
• Strong verbal and written communication; ability to present to diverse audiences.
• Knowledge of Medicare, Medicaid, Medicare Advantage, and commercial insurance programs.
• Excellent interpersonal skills for collaboration with patients and multidisciplinary teams.
• Experience in coordinating discharge planning, community resources, and crisis interventions.

Responsabilités

• Conduct patient assessment interviews and coordinate discharge plans.
• Identify and connect patients with community resources and referrals.
• Maintain patient records and communicate critical information to the medical team.
• Keep professional certifications up to date through continuing education.
