Qualifications

• 1+ year of social work in a health care setting strongly preferred.
• Licensed Master Social Worker (LMSW) or higher license in the State of Texas.
• At least three (3) years of accounts receivable management experience in health care.
• Must be able to communicate verbally and in writing in English.

Responsibilities

• Communicates with health care team and reports patient data in a comprehensive and unbiased manner; explains discharge plan.
• Facilitates discharge planning activities with interprofessional team and maintains ownership of psychosocial assessments and treatment.
• Participates in comprehensive case management activities and identifies barriers to efficient patient throughput.
• Assists with ensuring discharge disposition is appropriate and coordinates transfers as needed.
• Completes assessments identifying social determinants of health and connects patients to community resources.
