Job summary

Houston Methodist is seeking a Social Worker I to provide clinical social work and discharge planning on a designated unit. You will partner with physicians and the interprofessional team to address psychosocial aspects of illness and facilitate smooth transitions between care settings.

The position requires a Master’s in Social Work and an LCSW or LMSW license. New graduates are welcome; internship and hospital experience are preferred as you grow in a hospital environment.

Qualifications

• Master's degree in social work from an accredited university.
• LCSW or LMSW license required.
• 0 years experience; hospital internship preferred; one year hospital social work preferred.

Responsibilities

• Collaborates with physicians and interprofessional health care team to address psychosocial issues.
• Provides discharge planning and coordination to ensure continuity of care.
• Performs psychosocial assessments and plans based on social work diagnoses.
• Maintains documentation of assessments, treatments and discharge plans.
• Engages with patients and families using clear, compassionate communication.
