Intermountain Health in Sandy, Utah is seeking a Social Work Care Manager I to perform psychosocial assessments, develop care plans with the care team, and provide crisis counseling as needed. Qualified candidates hold a Master of Social Work, current licensure as CSW/LCSW, strong communication, critical thinking, and the ability to navigate care transitions.

Qualificações

• MSW from an accredited program with licensure required.

Responsabilidades

• Assess patients for transition planning, mental health, substance use, and goals of care.
• Develop and monitor care plans addressing social determinants and resources.
• Provide brief individual, group, and family therapy; conduct psychosocial assessments.
• Identify mental, emotional, and behavioral disorders and connect to services.
• Train staff, educate patients, and advocate for care access.
• Collaborate with healthcare teams, insurers, and community providers.
