Intermountain Health St Vincent Regional Hospital is seeking a Social Work Care Manager I to perform psychosocial assessments, develop care plans, and provide crisis counseling within a collaborative care team.

The role involves individual and family treatment, care coordination across providers, and advancing goals of care while addressing social determinants of health. 3x12 hour shifts are possible.

Qualifications

• MSW from an accredited institution with degree verification.
• Current state licensure before/upon supervision hours completion.
• Basic computer proficiency with common office software.
• Strong written and verbal communication skills.
• BLS certification, driver's license, and reliable transport for home visits.

Responsibilities

• Assessment & Screening: evaluates patients for transition planning, mental health, substance use, and goals of care.
• Care Coordination: develops and monitors care plans and connects to resources.
• Behavioral Health Support: uses motivational interviewing and therapeutic techniques.
• Therapeutic Intervention: provides brief individual, group, and family therapy.
• Diagnosis & Referrals: identifies disorders and connects to services.
• Education & Advocacy: trains staff and facilitates safe transitions of care.
• Team Collaboration: works with care teams, insurers, and community providers.
• Quality & Compliance: leads improvement initiatives and tracks metrics.
• Advanced Care Planning: facilitates goals of care discussions.
