Job summary

Qualifications

• Master of Social Work (MSW) from an accredited institution; degree verification required.
• Current state licensure, as applicable, obtained prior to or upon completion of required supervision hours.
• Basic computer proficiency with word processing and spreadsheets.
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking.
• Caregivers who conduct home/community visits must maintain BLS, have a valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Assesses patients for transition planning, mental health, substance use, and goals of care.
• Develops and monitors care plans, addressing social determinants and resources.
• Provides brief individual, group, and family therapy as needed.
• Identifies mental health disorders and connects patients to services.
• Trains staff, educates patients, and advocates for care access and safe transitions.
• Collaborates with care teams, insurers, and community providers for quality care.
• Leads improvement initiatives and ensures policy adherence.
• Facilitates goals-of-care discussions with patients, families, and teams.
