Qualifications

• Master's degree in social work required.
• Licensed Master Social Worker in the state of Michigan required.
• MSW - Limited license in Michigan required.

• Coordinate care management and behavioral health interventions.
• Develop strategies for cost-effective, quality care for complex patients.
• Identify levels of care and collaborate with patients, families, and providers.
