Job summary

First Choice Home Health/Jet Health Inc. is seeking a full-time Licensed Clinical Social Worker to provide psychosocial assessments, counseling, and resource referrals for patients in Boise, ID and surrounding areas.

The role involves in-home care across age groups and coordination with an interdisciplinary team. Requirements include an MSW or PhD in Social Work, a current driver’s license with CPR certification, and at least 1 year of LCSW experience in healthcare (home health preferred).

Qualifications

• MSW or PhD in Social Work from an accredited school.
• Current driver’s license and CPR.
• At least 1 year of LCSW experience in health care; home health preferred.
• Strong knowledge of community resources and good interpersonal skills.

Responsibilities

• Performs ongoing clinical assessment of home health patients to identify psychosocial, financial, environmental and community resource needs.
• Develops and evaluates the plan of care in partnership with the patient, caregiver and team.
• Provides counseling and education to patients, families and caregivers as ordered.
• Collaborates with the interdisciplinary team to promote coordination of patient care.
