Responsibilities

• 30% Clinical Assessment: conduct interviews, chart reviews, and develop care and discharge plans.
• 30% Intervention Management: provide individual/family/group interventions and crisis intervention.
• 25% Population Management: coordinate clinical and financial outcomes with interdisciplinary teams; liaise with payers.
• 10% Documentation and Regulatory Compliance: thorough documentation in medical records and systems; ensure regulatory compliance.
• 5% Maintain awareness of community resources and facilitate linkages for patient care.
