Responsibilities

• Clinical Assessment: interview patients and families, review charts, and plan care including discharge.
• Intervention Management: develop intervention plans and provide counseling, crisis intervention, and education.
• Population Management: coordinate clinical and financial outcomes, liaise with payers, ensure timely reports.
• Documentation & Regulatory Compliance: document assessments and interventions per standards and regulations.
• Other duties: maintain awareness of local resources and facilitate linkages.
