Job summary

Minimum qualifications include a Master’s in Social Work and an LSW license in Ohio, with 1–2 years of LSW experience; associate degrees grandfathered for those hired before 2019, and a 40-hour work week on

Qualifications

Master's Degree in Social Work or a related field and LSW license in the State of Ohio.
1-2 years’ experience as an LSW, preferably in a patient-centric setting and knowledge of medical terminology and computer skills.
Licensed Social Workers with Bachelors of Social Work hired prior to 01/10/2019 are grandfathered into this job.
Associates hired after 01/10/2019 will need to meet current qualifications.

Responsibilities

Clinical Assessment: Interviews, chart reviews, and collaboration to assess biopsychosocial functioning and develop care and discharge plans.
Intervention Management: Develops and implements plans of intervention across individual, family, or group modalities.
Population Management: Coordinate clinical and financial outcomes with interdisciplinary teams and manage post-discharge resources and referrals.
Documentation and Regulatory Compliance: Thorough documentation in medical records per standards and regulations.
Maintain awareness of community resources and facilitate linkages for patient care.
