Job summary

Ohio State University Hospital is seeking an IRP-Social Worker (HS) in Care Management Services. The role involves psychosocial assessments, counseling, crisis intervention and discharge planning to support patients and families through medical care transitions.

Minimum qualifications include an MSW from an accredited program and Ohio licensure (LSW/LISW) or equivalent. Position is intermittent, 0.2 shift, on-site at University Hospital Doan Hall.

Qualifications

• Master of Social Work from an accredited Graduate School of Social Work.
• State of Ohio Social Work licensure (LSW or LISW) or equivalent education/experience.

Responsibilities

• Provide psychosocial assessments, mental health counseling, crisis intervention and discharge planning.
• Offer financial counseling, health education and linkage with community resources.
• Address medically related social and emotional needs impacting treatment, recovery and safe transitions.
