Qualifications

• Licensure by Social Work Certification and Licensure Board.
• Knowledge of medical terminology and computer skills.

Responsibilities

• Clinical Assessment: interviews, chart reviews, and care planning.
• Intervention Management: develop plans via individual/family/group modalities.
• Population Management: coordinate care and liaise with payers.
• Documentation: maintain records per departmental standards.
• Stay informed on community resources and linkages for patients.
