What You’ll Do

• Receive case referrals, review patient information, and assign clinicians accordingly.
• Evaluate cases by reviewing services provided, conducting conferences, record reviews, and guiding clinicians to improve performance.
• Review patient diagnoses, medications, procedures, and clinical course.
• Assist clinicians in establishing therapeutic goals and developing care plans.
• Conduct or assist with quarterly record reviews and communicate findings to leadership.
• Contribute to policy, procedure, and strategic goal development and evaluation.

Qualifications

• Licensed clinician with current licensure in the state.
• Graduate of an accredited school; BSN/DPT/MOT/MSLP preferred.
• Minimum of two years management or supervisory experience in healthcare, preferably home care.
• Current CPR certification.
• Valid driver’s license and proof of insurance.
