What You’ll Do:

• Receive case referrals, review patient information including disciplines required to determine home care needs, and assign appropriate clinicians to cases.
• Review and evaluate each case by assessing services provided by clinicians, conducting conferences, reviewing records, verifying impressions, and guiding clinicians to ensure effective performance and quality home care services. Be available during operating hours to assist clinicians as needed.
• Review patient’s clinical diagnosis, medications, procedures, and clinical course.
• Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care.
• Conduct or assist with quarterly record reviews, communicate findings and recommendations to the Executive Director and relevant personnel.
• Help formulate, revise, implement, and evaluate policies, procedures, and strategic goals and objectives.

Qualifications:

• Licensed clinician with current licensure to practice within the state.
• Graduate of an accredited school; BSN/DPT/MOT/MSLP degree preferred.
• At least two years of management or supervisory experience in a healthcare setting, preferably in home care.
• Current CPR certification.
• Valid driver’s license and proof of insurance.
