This position is bonus eligible, and you can earn up to 15% of your annual salary!

What You’ll Do:

• Receive case referrals, review patient information to determine home care needs, and assign clinicians accordingly.
• Evaluate cases by reviewing services provided, conducting conferences, record reviews, and guiding clinicians to ensure quality home care services.
• Review patient diagnoses, medications, procedures, and clinical course.
• Assist clinicians in establishing therapeutic goals, setting priorities, and developing care plans.
• Conduct quarterly record reviews and communicate findings to the Executive Director and staff.
• Contribute to the formulation, revision, and implementation of policies, procedures, and strategic goals.

Qualifications:

• Licensed clinician with current licensure within the state.
• Graduate of an accredited school; BSN, DPT, MOT, or MSLP degree preferred.
• At least two years of management or supervisory experience in healthcare, preferably home care.
• Current CPR certification.
• Valid driver’s license and proof of insurance.
