This position is bonus eligible, with the potential to earn up to 15% of your annual salary!

Responsibilities
  1. Review case referrals and patient information to determine care needs and assign clinicians.
  2. Evaluate clinical services provided, guide clinicians, and promote quality home care services.
  3. Review patient diagnoses, medications, procedures, and clinical course.
  4. Assist clinicians in establishing therapeutic goals and developing care plans.
  5. Conduct or assist with quarterly record reviews and communicate findings.
  6. Contribute to policy and procedure development, and strategic planning.
Qualifications
  • Licensed clinician with current licensure in the state.
  • Graduate of an accredited school; BSN/DPT/MOT/MSLP preferred.
  • Two years of management or supervisory experience in healthcare, preferably home care.
  • Current CPR certification.
  • Valid driver’s license and proof of insurance.
