This position is bonus eligible, with the potential to earn up to 15% of your annual salary!

What You’ll Do
  1. Receive case referrals, review patient information, and determine home care needs. Assign appropriate clinicians to cases.
  2. Review and evaluate clinical services, conduct conferences, record reviews, and guide clinicians to ensure quality home care services.
  3. Review patient diagnoses, medications, procedures, and clinical course.
  4. Assist clinicians in establishing therapeutic goals, setting priorities, and developing care plans.
  5. Conduct quarterly record reviews and communicate findings to leadership.
  6. Assist with policy and procedure development, revision, and strategic planning.
Qualifications
  • Licensed clinician with current licensure in the state.
  • Graduate of an accredited school; BSN/DPT/MOT/MSLP preferred.
  • Minimum of two years management or supervisory experience in healthcare, preferably home care.
  • Current CPR certification.
  • Valid driver’s license and proof of insurance.
