Personal Touch Home Aides of New York is seeking a Clinical Scheduler and Support Specialist to coordinate and schedule field clinicians, maintain patient records in EMR, and assist the nursing department with audits.

The role requires NYC borough knowledge, strong computer skills (Excel, Word, PDF editing, EMR), attention to detail, and the ability to manage multiple priorities in a fast-paced home-care environment.

Qualifications

• Strong knowledge of NYC boroughs and neighborhoods to cluster visits.
• Proficient in Excel, Word, PDF editing and EMR.
• Ability to manage spreadsheets and enter data accurately.
• Fast and accurate typing skills.
• Scan, upload, and organize documents efficiently.
• Excellent communication and professional etiquette.
• Experience in home care CHHA or LHCSA preferred.

Responsabilités

• Coordinate clinician visit schedules and track SOC, supervisory, preceptorship and clinical follow-up visits.
• Staff referrals and active patients; maintain spreadsheets for active patients and service areas.
• Cluster visits geographically using NYC boroughs, neighborhoods, and travel patterns.
• Communicate with field clinicians and managers to ensure smooth workflow and timely service delivery.
• Administrative support to the clinical department: organize charts, upload documents, manage deadlines.
• Assist with internal and external audits (DOH, OMIG, MLTC) by gathering documentation and visit logs.
• Monitor email queues and task lists to ensure timely responses; provide excellent patient/family service.
• Perform other duties to support clinical operations.
