Personal Touch Home Care is seeking a Scheduler and Clinical Support Specialist to coordinate clinician visits and maintain patient records in EMR. The role requires strong communication, organizational skills, and knowledge of NYC boroughs and travel patterns.

Qualifications

• Must be detail-oriented with strong organizational skills.
• Experience coordinating clinician visits and scheduling in a CHHA/HH context preferred.
• Proficient in NYC boroughs and travel patterns.

Responsibilities

• Coordinate and track clinician visits, including SOC and supervisory visits.
• Staff new referrals and maintain patient service area spreadsheets.
• Cluster visits geographically using NYC borough knowledge.
• Communicate with field clinicians and managers to ensure smooth workflow.
• Provide administrative support to the clinical department.
• Assist with internal and external audits and gather documentation.
• Monitor email queues and manage task lists.
• Provide excellent customer service to patients, families and clinicians.
• Perform other duties to support clinical operations.

Job Summary

Our Certified Home Health Agency (CHHA) is seeking a highly organized Scheduler and Clinical Support Specialist to support our clinical operations. This role is responsible for coordinating and scheduling field clinicians visits, maintaining accurate and up-to-date patient records in EMR and assisting the nursing department with audit preparation and compliance activities. The ideal candidate is detail-oriented, proactive, good communicator, and comfortable working in a fast-paced environment with multiple priorities and working knowledge of New York City geography—including boroughs, neighborhoods, and travel patterns. They must be confident using technology, running reports, managing spreadsheets.

Responsibilities

• Coordinate the schedules, confirm, and track all clinician visits, including SOC, supervisory, preceptorship and clinical follow-up visits.
• Responsible for staffing all new referrals and active patients maintaining all spreadsheets for active patients and service areas.
• Clustering visits geographically using strong knowledge of NYC boroughs, neighborhoods, and travel patterns
• Communicate with field clinicians and managers to ensure smooth workflow and timely service delivery.
• Serve as administrative support to the clinical department, including organizing charts, uploading documents, and managing deadlines.
• Assist with internal and external audits (DOH, OMIG, MLTC), including gathering documentation, compiling visit logs, and ensuring data accuracy.
• Monitor email queues and task lists to ensure timely responses to inquiries and requests.
• Provide excellent customer service when interacting with patients, families and clinicians.
• Perform other duties as assigned to support clinical operations.

Desired Skills and Experiences

• Strong working knowledge of NYC boroughs, neighborhoods, and surrounding areas to efficiently cluster and route clinician visits.
• Strong computer proficiency (Excel, Word, PDF editing, EMR).
• Ability to manage spreadsheets, create simple trackers, and handle data entry with high accuracy.
• Fast and accurate typing skills.
• Ability to scan, upload, and organize documents logically and efficiently.
• Excellent communication skills and professional phone/email etiquette.
• Strong attention to detail and ability to manage multiple priorities.
• Experience in home care, CHHA (preferred) or LHCSA.
