Job summary

Penn Medicine is seeking an experienced RN to join our utilization management and discharge planning team. The role coordinates home care, DME, hospice and community resources, ensuring timely communication with the interdisciplinary care team.

You will educate patients on coverage and benefits, document in the Canopy system, and support families through transitions, while upholding patient privacy and dignity.

Qualifications

• Registered Nurse – PA license required.
• Bachelor's degree or higher.
• Minimum 2 years of clinical experience.

Responsibilities

• Monitor appropriate use of internal resources and coordinate with care teams.
• Coordinate discharge planning, home care, DME, hospice, and community resources.
• Collect quality data and manage risk referrals.
• Educate patients and families on health care benefits and privacy.
• Maintain up-to-date knowledge of new providers and treatment modalities.
• Assist patient with transfers to appropriate next level of care.
