As an LPN, you'll play an essential role in ensuring our residents receive safe, compassionate, and individualized care. Working under the direction of a Registered Nurse, you will:
Provide hands-on nursing care while promoting resident comfort, independence, and dignity.
Administer medications and treatments as prescribed, including intravenous services within your authorized scope of practice.
Monitor residents for changes in condition, document findings, and communicate concerns promptly to the appropriate clinical team.
Coordinate daily resident care by assigning tasks to Certified Nursing Assistants (CNAs), monitoring completion, and providing guidance and support.
Develop and update individualized care plans that reflect residents' needs, preferences, and personal goals.
Collaborate with nurses, physicians, caregivers, residents, and families to support positive care outcomes.
Maintain accurate clinical documentation and follow established nursing procedures and infection prevention practices.
Advocate for residents' rights, choices, and quality of life.
We're looking for an LPN who brings clinical knowledge, compassion, and a team-oriented approach to resident care.
Required Qualifications
Graduate of an accredited School of Practical Nursing.
Current, active, and unencumbered Oregon LPN license.
Strong nursing assessment, clinical judgment, and documentation skills.
Ability to recognize and respond appropriately to changes in resident condition.
Excellent communication, organizational, and interpersonal skills.
Commitment to resident-centered care, professionalism, and ethical nursing practices.
Ability to work collaboratively in a fast-paced healthcare environment.
Preferred Qualifications
Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric nursing.
Experience using Electronic Medical Records (EMR).
Experience coordinating care and providing guidance to CNAs.

This is an on-site position at our Portland campus. The role involves regular walking and standing, resident care activities, lifting and moving supplies, and other physical requirements associated with nursing in a skilled nursing environment.
