Job summary

WellSpace Health, Inc. seeks a Wellness Ambassador to join the Quality team. You will engage patients to close care gaps, coordinate with clinical and operational staff, and help improve HEDIS and UDS performance in WellSpace Centers.

Primary responsibilities include real-time patient engagement, care coordination, and expertise in quality workflows. Requires strong communication, HIPAA compliance, and ability to work with diverse populations.

Qualifications

• High school diploma or equivalent required; bachelor’s preferred in public health or related field.
• Experience in primary care settings or equivalent education/experience.
• Experience in community engagement and patient education required; Motivational Interviewing preferred.
• Ability to communicate clearly, with strong listening and confidentiality adherence.

Responsibilities

• Support data collection and reporting for the Quality plan.
• Assist in using Health Maintenance and EHR tools to improve workflows.
• Identify quality improvement opportunities and assist in data analysis.
• Coordinate care gap closure with care teams and patient engagement.
