Job summary
Fairview Health Services seeks a seasoned Pharmacist to oversee clinical pharmacy programs, the IVIG Stewardship Program, and infusion-related consulting to optimize patient care and payor strategy across infusion centers.
The role involves collaboration with support staff, adherence to policies, and leadership in medication therapy management for home and infusion center IV therapies. Requires BS PharmD and MN licensure, with 1–3 years in specialty/oncology infusion meds.
Qualifications
- Bachelor of Science Pharm D in Pharmacy from an accredited school of pharmacy.
- 1–3 years experience in Specialty or Oncology infusion medication.
- MN Pharmacist Licensure Upon Hire.
Responsibilities
- Provide pharmaceutical care including patient counseling and drug use evaluation for home IV therapy.
- Develop and manage a clinical program addressing payor concerns for Immunoglobulin therapy in infusion centers.
- Consult with infusion finance securing teams for prior authorizations and clinical reviews for IV therapy patients.
- Monitor payor landscape and adjust programs for changes in coverage.
